We facilitate care coordination by serving as a vital link between the recipient, the home health agency (caregivers), and the health insurance provider. If concerns arise or assistance is needed, we advocate on behalf of elderly and disabled individuals. We also conduct monthly check-in calls to ensure ongoing support and address any needs

We assist eligible individuals with enrollment in the New York State Nursing Home Transition and Diversion (NHTD) Program. Our team guides recipients and their families through each step of the eligibility and application process, helping them understand program requirements, complete necessary documentation, and coordinate with the appropriate agencies. Our goal is to ensure a smooth transition into the NHTD Program, enabling individuals to receive the supports and services needed to live safely and independently in the community.

If eligible, we provide recipients with guidance on navigating the process of increasing home care hours. Starting with them communicating with their respective care managers at their health insurance plans, guiding through the process of obtaining paperwork from their healthcare providers and submitting them to health insurances, if necessary.
We will provide information and assistance within HIPPA/lawful limits in the process of Fair Hearings, if needed.

Children over the age of four with qualifying medical conditions plus Medicaid may also be eligible for home care services. We provide guidance throughout the process of obtaining these services, helping to ease the burden and reduce stress for parents and families.
